Benefits Summary for Providers:

(Effective January 1, 2016)

This information highlights the current PCHC standard benefits as of January 1, 2016. Please refer to the complete Employee Handbook for details of each program. Benefits are subject to change by PCHC, Inc.

BCBS Vantage Blue Health Insurance  *
Coverage begins the 1st day of the month following your date of hire.
Payroll deductions for health insurance are:

$11.00 / week for an individual plan – 21+ standard hours (0.6 FTE) to be eligible
$22.00/week for Individual Plus 1 28+ standard hours (0.8 FTE) to be eligible
$ 33.00 / week for a family plan – 28+ standard hours (0.8 FTE) to be eligible

*Child covered until the end of the month they turn age 26.

Family Health Insurance Waiver – 28+ standard hours (0.8 FTE) to be eligible
If you are currently enrolled in a family health insurance plan, you may waive PCHC coverage and receive $75.00 per week. This amount is added to your gross salary and paid out in your weekly paycheck. You will be required to submit acceptable documentation of other insurance. Employees are not entitled to a Health Insurance waiver if they receive health insurance from PCHC via a spouse or parent.

Individual Plus One Health Insurance Waiver – 28+ standard hours (0.8 FTE) to be eligible
If you are currently enrolled in a family health insurance plan, you may waive PCHC coverage and receive $50.00 per week. This amount is added to your gross salary and paid out in your weekly paycheck. You will be required to submit acceptable documentation of other insurance. Employees are not entitled to a Health Insurance waiver if they receive health insurance from PCHC via a spouse or parent.

Individual Health Insurance Waiver – 21+ standard hours (0.6 FTE) to be eligible
If you are currently enrolled in an individual health insurance plan, you may waive PCHC coverage and receive $25.00 per week. This amount is added to your gross salary and paid out in your weekly paycheck. You will be required to submit acceptable documentation of other insurance. Employees are not entitled to a Health Insurance waiver if they receive health insurance from PCHC via a spouse or parent.

Dental Insurance * (Regular part time and full time employees)
PCHC offers dental coverage through Delta Dental of RI, which is 100% employee-paid. If you choose to subscribe, coverage will begin the first day of the month following your date of hire. You may choose from a standard plan or a high-option plan.

Payroll deductions for the standard plan are:
$ 7.31/ week for an individual plan;
$ 14.63/ week for an individual plus 1
$ 26.54/ week for a family plan

Payroll deductions for the high option plan are:
$ 9.42/ week for an individual plan;
$ 18.84/week for an individual plus 1
$ 31.31/ week for a family plan

*Children covered until December 31st, the year they turn 26.

Vision Insurance (Regular part time and full time employees)
PCHC offers vision coverage through VSP, which is 100% employee-paid. If you choose to subscribe, coverage will begin the first day of the month following your date of hire.

Payroll deductions for vision insurance are:
$ 1.85/ week for an individual plan;
$ 2.69/ week for an individual plus 1
$ 4.82/ week for a family plan

*Children covered until December 31st, the year they turn 26.

Pre-tax Reduction Agreement
Individuals who enroll in the PCHC Health, Dental, Vision, Cancer, and/or Accident plan(s) may choose to have these payroll deductions taken out on a pre-tax basis.

Health Reimbursement Account (HRA)
The HRA is available to all employees who enroll in the PCHC medical insurance plan. Certain co-payments and deductibles are reimbursed in whole, or part, by PCHC.

Flexible Spending Accounts (FSA) – 17.5+ standard hours (0.5 FTE) to be eligible

Health Care Flexible Spending Account
You may enroll to set aside pre-tax dollars each pay period to pay for health-related expenses such as co-payments, un-reimbursed dental expenses, prescriptions and/or certain over the counter medications, eyeglasses, deductibles, etc. A VISA Debit Card is provided to participate to use to pay for eligible items. You are eligible for enrollment on the 1st day of the month following your date of hire.

Dependent Care Flexible Spending Account
You may enroll to set aside pre-tax dollars each pay period to pay for day care, pre-school, summer day-camps, elder care, etc. for eligible dependents. You are eligible for enrollment on the 1st day of the month following your date of hire.

Voluntary Benefits which include a Wellness Incentive Benefit offered from TRANSAMERICA
17.5+ standard hours (0.5 FTE) to be eligible

Cancer – Cancer Insurance offers unique Wellness Benefits that can provide the motivation employees need to be involved in their own health affairs. In the event of a cancer diagnosis it offsets out-of-pocket expenses. The benefit is paid directly to the insured for cancer related expenses. Coverage levels available include: employee, employee plus child(ren), and family. It has the option of being maintained on a direct bill if you were to leave employment.

Accident Program Having good health is one of your most important assets. The accident program can provide you with wellness benefits.  In the event of an accident it will provide direct payment to help offset uncovered expenses. Other benefits include: Enhanced Emergency Room benefits, Follow-up, Hospital Confinement, and Physical Therapy payments. Coverage levels available include: employee, employee plus child(ren), employee plus spouse and family.

Supplemental Life Insurance [27+ standard hours or more]
Employee, spouse and/or child(ren) may voluntarily enroll in a life insurance plan offered by Lincoln Financial. If you enroll in the plan, you are eligible to also purchase life insurance coverage for your spouse and/or child (ren).

Retiree Medical Benefits
PCHC extends individual health insurance benefits to employees who retire at age sixty-two (62), sixty-three (63), and sixty-four (64), with twenty (20) or more years of service. Insurance will continue until they reach age 65.

Tax Sheltered Annuity Retirement Plan – 403B (Automatic Enrollment)
You will automatically be enrolled to defer one percent (1%) of your eligible pay each pay period, starting with your first paycheck, unless you complete a declination form. You are fully-vested from day one of your employment, if you are enrolled in the plan. PCHC matches your deferral contribution up to five percent (5%) of your gross annual salary, for all regular employees, after six (6) months of employment, if you are a plan participant (excludes temporary and per diem staff).

Life Insurance [ 27 standard hours or more]
All regular, providers with 27 standard hours or more are eligible for company-paid Life Insurance coverage, effective the first day of the month following their date of hire.

Short-term Disability (STD) Insurance  [27 standard hours or more]
In case of a confirmed/documented disability, your company-paid STD benefit will provide you with financial assistance during your absence. Benefits begin after a 15-day waiting period, for both accident and sickness, and will be paid for up to 11 weeks.

Long-term Disability (LTD) Insurance [ 27 standard hours or more]
In case of a confirmed /documented disability beyond ninety (90) days, LTD insurance will provide you with financial assistance. As is the case of STD, this benefit is also fully-paid by PCHC.

Pre-Taxed LTD [ 27 standard hours or more]
You will have the option to choose whether or not to pay the taxes on the premium now or on the benefit later. If you do not pay the taxes on the premium now (through payroll), then you will need to pay the taxes on the benefit, should you ever begin drawing the benefit. If you do choose to pay the taxes on the premium (this will be handled by HR and payroll), then the benefit, should you ever need it, will not be taxable.

Coastline Employee Assistance Program (EAP)
Work/life services are available to you or an immediate household family member 24 x 7 x 365 for matters such as: parenting and childcare, eldercare, relationships, work and career, and financial matters. The EAP provides in-person sessions for short-term problem resolution, as well as referrals to qualified resources.

CME (Continued Medical Education)
Five (5) days of paid leave and $2,000 per year, prorated based upon standard hours

Accrued Vacation Time*
You will accrue and may use three weeks per year for the first three years of employment and four weeks/year in the fourth year and thereafter.  Time off is calculated in standard hours as follows:

  • Three weeks/year (for first 3 years)       3 x # of standard hours
  • Four weeks/year (after 3 years)              4 x # of standard hours

Sick Time*
Six (6) days, pro-rated based upon standard hours

Personal Time (PL)*
Two (2) days of paid leave, prorated based upon standard hours

* These benefits are accrued by standard agency formulas, and are based on clinical patient contact hours (standard hours). During the first year of employment, vacation, sick time and PL time accruals are prorated base upon date of hire.

Bonus Plan
An incentive bonus program is in place and will be reviewed with you in detail during your orientation process.

Other Benefits
PCHC pays for all hospital dues, required medical licenses, the license application fee for new physicians and dues for membership in one professional organization.

Holiday
PCHC recognizes 11 paid holidays each year. You will be paid holiday hours upon the regularly scheduled hours that you would have worked. To be paid for a holiday, an employee must work the scheduled day before and after the holiday or have a pre-approved leave with pay, such as vacation or PL time.

Jury Duty
Employees who are called to Jury Duty must show verifying documentation to their supervisor immediately upon receipt of notification. For the first four (4) weeks, PCHC will pay you the difference between the amount received each day for Jury Duty and the amount you would have earned had you worked.

Bereavement
Employees may be granted time off with pay not to exceed five (5) consecutive days (pro-rated for part-time employees) following the death of an immediate family member. Employees must provide an obituary notice for verification to their manager and to Human Resources.

Direct Deposit
Employees will be offered the opportunity to have their paycheck directly deposited in the account of their choice.

Longevity Bonus
Employees receive a longevity bonus on their anniversary date of hire based on the completion of 10 + years.
