We Strive for Excellence

  • We are certified as a Level 3 (the highest) Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).

What is a Patient-Centered Medical Home?

“You and your health care are at the center of the medical home team.”

A patient-centered medical home is health care focused on the patient. It is a partnership between the patient and medical home team, including the primary care doctor. As patients require additional services from specialists and other healthcare providers, they’ll become part of the patient’s medical team as well. Therefore, PCMH is made up of health care professionals committed to improving the patient’s overall health and helping the patient reach health goals.

  • A Patient Portal is fully available to patients in order to assist them to increase their involvement in their own care. We have fully implemented the Electronic Health Records (EHR) which help our providers and patients work together and, most importantly, help our providers deliver the best possible care.
As we grow and innovate, we continue our commitment to improve the health outcomes of all the patients we serve!
